Because the kid’s heads are so BIG!
I mean, look at Stewie. He’s got a head shaped like a football. Lois’ pelvis would not be able to accommodate such an object.
There’s a reason we chose those two for the picture, but babies in general have head far too big for the relatively narrow hips of cartoon mothers.

That’s exactly it. Who would have thought that cartoons are not biologically accurate?
